The forms at the left are provided for your convenience.  Having them completed before arriving on registration day will help speed up the process of enrolling your student.  

For students who are new to the district, please bring these additional items:

• Personal Information Sheet

• Student Record Release

• Home Language Survey

• Proof of residency (see board policy)

• Student in Good Standing Transfer Form

• Copy of current physical and immunization

• Certified copy of Birth Certificate

• Test Scores (Achievement, ISAT, PSAE as appropriate)

• Transcripts from previous school

• Social Security Number
